Healthcare Program
In addition to primary care, we offer screening, prevention, education, and chronic disease management programs. By working with families to help them understand how to manage chronic conditions and to promote healthy life styles, our goal is to reduce the costly cycle of hospital emergency department visits and preventable hospitalizations. During 2007 we supported approximately 2400 patient visits.
Disease Management Programs Offered
- Hypertension Management: In collaboration with MOTTEP and Howard University, we are enrolling clients in a hypertension management program. Download an information flyer here or contact our office for more information.
- Diabetes Support Group: Groups meet on the first Thursday of the month; one group 10 AM - Noon; one group 6 - 8 PM. Download an information flyer here or contact our office for more information.
Health Education Classes
Health education classes are offered almost every month. These are free and presented by experts. Watch our web site home page or call our office for class announcements. Some past classes have been:

Other Specific Programs Available
- Mobile Pediatric Clinic: The Children's Hospital of Michigan CATCH mobile pediatric clinic operates at Joy-Southfield on the 2nd & 4th Tuesdays from 9AM to 3PM. Call 313-966-0626 for an appointment. This care is free for insured and uninsured.
- If you are an African American adult age 21 or over with type 2 Diabetes, you may be interested in participating in a Long-Term Self- Management and Support groups. Call the University of Michigan Diabetes Study Line at 1-800-529-5345 for more information.
- Women (ages 40-64) can receive free mammograms, PAP's and follow-up through Karmanos Hospital Breast and Cervical Cancer Control Program, or call 888-242-2702.
- HIV/AIDS, breast & cervical cancer screening & counseling: call our office for the schedule.
Health Education Survey Report
During the spring and summer 2008, Joy-Southfield CDC conducted a health education survey of West-side Detroit residents, including the youth and families of the Cody-Rouge neighborhood. Valuable information was obtained that will help us design our preventive education programs for maximum benefit. A summary of this report is available here.
